You will be rescheduled if the form is not completed before the provider calls you. ​

Please indicate if you have Facetime or Google Duo on your phone. You will be called on one of these formats. 

Occasionally, the provider will have an NP student present during your visit.

If you do not answer, you will be rescheduled for another day.  YOU are responsible to make sure we have your updated phone number and email address.
